Eligibility: NO PURCHASE NECESSARY TO ENTER OR WIN. PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. Contest is open only to residents of the United States, the District of Columbia and United States Territories who are 18 years of age or older as of April 1, 2010. To be eligible to win a prize, entries must be completed and received by Carlson Laboratories in the format and in accordance with the rules designated below. Employees, officers, and directors of Carlson Laboratories, www.carlsonlabs.com, and their respective affiliates, subsidiaries, parent and related companies, and agencies and individuals living in the same household (whether or not related) or immediate family members (spouses, parents, children and siblings, regardless of where they live) of such employees, officers or directors, are not eligible to enter the contest. Food professionals, such as chefs, food writers or recipe developers who create recipes for pay, are ineligible to win a prize. All applicable federal, state and local laws and regulations apply. Offer void where prohibited or restricted by law. Limit one (1) entry per person.

How to Enter Contest: Submit original recipe and recipe photo either: (1) using the online form available at www.carlsonlabs.com/recipecontest, or (2) by mailing submission, including full name, address, city, state, zip code, telephone number and e-mail address (if you have one) to: Carlson Laboratories Attn: Fish Oil Recipe Contest, 15 College Drive, Arlington Heights, IL 60004.
Recipe entries must be submitted or postmarked by the Deadline. Only entries actually received by Carlson Laboratories or www.carlsonlabs.com will be entered. Received
means actually delivered to Carlson Laboratories or www.carlsonlabs.com by the U.S. Postal Service or another delivery service, or for online entries, when www.carlsonlabs.com's servers record the entry information. Any automated computer receipt (such as one confirming delivery of e-mail) does not constitute proof of actual receipt by Carlson Laboratories of an entry for purposes of these Official Rules. In the event of a dispute about the identity of an online entrant, each online entry will be declared made by the authorized account holder of the e-mail address submitted at the time of entry. Authorized Account Holder
is defined as the natural person who is assigned to an e-mail address by an Internet Access Provider, online service provider, or other organization (e.g. business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. If the winning entry is an online entry, the potential winner may be required to provide proof that s/he is the authorized account holder of the email address associated with the winning entry.
When entering online, participants may choose to opt in to newsletters and special offers from Carlson Laboratories. Participant names will be activated within thirty (30) days of submission. Participants are not required to opt in and participants may opt out from Carlson Laboratories newsletters and special offers at any time.
Contest Rules: Create your own original recipe using Carlson Liquid Norwegian Fish Oil. All Recipes must include at leasttwo (2) Tablespoonfuls of Carlson Liquid Norwegian Fish Oil and serve four (4) people. You cannot cook with fish oils because heat will damage the fragile oils. Fish oil can be added after cooking when the food has cooled. Recipes must be original. Carlson Laboratories will disqualify any recipe that (to our knowledge) has been previously published or that is based on any other published recipe. Each original recipe must include a list of all required ingredients in specific, common U.S. household measurement units, step-by-step preparation instructions, and cooking times (if applicable). In the event two or more participants enter the same recipe, the entry which was received by Carlson Laboratories first will be entered in the contest and the duplicate recipe(s) received by Carlson Laboratories at a later time or date will be disqualified without notice to the entrant.
Judging and Selection: After the Deadline, a judge selected by Carlson Laboratories will choose 10 recipes from the qualified entries based upon the evaluation criteria set forth below (Finalist Recipes
) to be judged in a final round. After the Finalist Recipes have been chosen, a panel of judges selected by Carlson Laboratories will judge the Finalist Recipes based on the following evaluation criteria: 100-point scale based on healthfulness (0-20 points), taste (0-20 points), creativity (0-20 points), aesthetics (0-20 points) and practicality (0-20 points). Practicality will be based on efficiency, ease of preparation, expense and availability of ingredients. Decisions of the Carlson Laboratories judges are final and may not be appealed. Winners will be selected on or about September 30th, 2010 and notified via telephone or e-mail. Winners must respond within five (5) business days of the first attempted notification following the instructions contained in the notification. Failure of a winner to timely respond to Carlson Laboratories' attempted notification, failure to timely respond to instructions contained in the notification, including without limitation, at the request of Carlson Laboratories, the submission of completed affidavits, releases and publicity releases, or if otherwise determined to be ineligible to win a prize in the sole discretion of Carlson Laboratories, will result in disqualification from the contest and Carlson Laboratories will choose an alternate winner.
Prizes: Three (3) prizes will be awarded: (1) The grand-prize winner will receive $500.00; (2) the 2nd place winner will receive $200.00; and (3) the 3rd place winner will receive $100.00. Odds of winning a prize depend on the number of eligible entries received and the quality of the entries received. ALL FEDERAL, STATE AND LOCAL TAXES ASSOCIATED WITH THE RECEIPT OF ANY PRIZE IS THE SOLE RESPONSIBILITY OF THE WINNER. Each prize returned as undeliverable or otherwise not claimed within five (5) days after first attempted delivery will be forfeited and awarded to an alternate winner. Carlson Laboratories shall not be required to replace any lost or stolen prizes. Any expenses not specifically stated are the sole responsibility of the winner.
Carlson Laboratories' Ownership of Entries: All entries and materials submitted to Carlson Laboratories in connection with the contest (Entry Materials
), along with all copyright, trademark, and other proprietary rights associated therewith, become the property of Carlson Laboratories upon submission, and Entry Materials will not be returned to any entrant. Without limiting the generality of the foregoing, you acknowledge that Carlson Laboratories will own all rights to use, modify, reproduce, publish, perform, display, distribute, make derivative works of and otherwise commercially and non-commercially exploit the Entry Materials in perpetuity and throughout the universe, in any manner or medium not existing or hereafter developed, without separate compensation to you or any other person or entity. Carlson Laboratories' use of the Entry Materials may include, but shall not be limited to, publication in a Carlson Laboratories cookbook. You agree to take, at Carlson Laboratories' expense, any further action (including, without limitation, execution of affidavits and other documents) reasonably requested by Carlson Laboratories to effect, perfect or confirm Carlson Laboratories' ownership as set forth in this paragraph.
Original Work of Authorship: By submitting an entry, you warrant and represent that (a) you own all rights to all Entry Materials submitted by you; and (b) all such Entry Materials are original works of authorship on your part and have not been copied, in whole or in part, from any other work and do not violate, misappropriate or infringe any copyright, trademark or other proprietary right of any other person or entity.
General Release: By entering the contest, you agree to release Carlson Laboratories, www.carlsonlabs.com, and their respective agents, officers, directors and employees from any and all liability whatsoever, and waive any and all causes of action related to any claims, costs, injuries, losses, or damages of any kind arising out of or in connection with participation in this contest or with respect to awarding, receipt, possession, delivery, misdelivery, use and/or misuse of any prize.
Use of Winner's Name, Likeness, Etc.: Except where prohibited by law, entry into the contest constitutes permission to use winners' name, address, likeness and/or prize information, without limitation, for promotional purposes without further permission or compensation. As a condition of being awarded any prize, except where prohibited by law, winners may be required to execute and deliver to Carlson Laboratories a consent to use of their name, address, likeness and/or prize information, without limitation, for promotional purposes without further permission or compensation.
Affidavit and Release: As a condition of being awarded any prize, winners may be required to execute and deliver to Carlson Laboratories a signed affidavit of eligibility and acceptance of these Official Rules and release of liability.
Winners List and Rules Request: For a copy of the winners list, send a stamped, self-addressed, business-size envelope after October 1, 2010 and before October 1, 2011 to Carlson Laboratories at the address listed above. Miscellaneous: The contest and these Official Rules will be governed, construed and interpreted under the laws of the state of Illinois without regard to choice of law provisions. Entrants agree to be bound by these Official Rules and by the decisions of Carlson Laboratories, which are final and binding in all respects. Carlson Laboratories reserves the right to change these Official Rules at any time, in its sole discretion, and to suspend or cancel the contest or any entrant's participation in the contest should viruses, bugs, unauthorized human intervention or other causes beyond Carlson Laboratories' control affect the administration, security, or proper play of the contest or Carlson Laboratories otherwise becomes (as determined in its sole discretion) incapable of running the contest as planned. Entrants who violate these Official Rules, tamper with the operation of the contest, or engage in any conduct that is detrimental or unfair to Carlson Laboratories, the contest, or any other entrant (in each case as determined in Carlson Laboratories' sole discretion) are subject to disqualification from entry into the contest.
